Fees for Paper Copies
Paper copies (in place of free digital records) will incur a cost, depending on the number of pages:
- Initial $5 fee for up to 200 pages of medical records
- Per page fee of $0.10 after the initial 200 pages
- Imaging disk is $5 per copy
